Forms, intake and surveys.

Intake, order and feedback forms that fill in your records on their own. Nobody retypes a clipboard, and an unhappy answer reaches you the same day.

The record is in SimpleUP CRM. Chris walks in and goes straight to the room.
Example with made-up names

Every clipboard gets typed in twice.

Good information, before the customer arrives.

Paper forms and back-and-forth emails mean someone retypes the same details, often with mistakes. A short form that lands straight in your records saves that time, and you walk in prepared.

Without it

  • Clipboards at the front desk
  • Details retyped from email into the system
  • Missing information found on the day
  • Feedback collected, then never read

With it

  • Forms filled in on their phone before they arrive
  • Answers saved to the right record automatically
  • Required questions can't be skipped
  • Unhappy answers sent to you the same day

What it looks like in real businesses.

Three kinds of form, and where the answers went.

Dental clinicMississauga · new patients
The form
New patients fill in health history and insurance on their phone after booking.
What happens Done before the visit
Answers land in the patient's record. The front desk only checks the insurance card on arrival.
Real estate agentBurlington · buyers
The form
A short buyer questionnaire: budget, areas, must-haves and pre-approval.
What happens Sorted
Each answer tags the buyer in SimpleUP CRM, so new listings that match go to the right people.
CateringWhitby · catering orders
The form
Office orders used to arrive by email, missing the headcount or dietary needs.
What happens Complete every time
The order form asks for date, headcount and allergies. The customer gets a confirmation and the kitchen gets the list.

How it works, step by step.

  1. 1

We build the forms with you

Intake, orders, applications, feedback or a short quiz. Your questions, your logo, and no longer than it needs to be.

  2. 2

    They go where customers are

On your website, in the booking confirmation, as a text or email link, or on a tablet at the counter.

  3. 3

    Answers fill in your records

Each answer lands in the right place in SimpleUP CRM. No copying, no retyping.

  4. 4

    The right people are told

A new order goes to the kitchen, a new patient to the clinician, a concern to you.

  5. 5

    Feedback is sorted for you

Happy, neutral and unhappy answers are grouped each week, and the unhappy ones are sent to you the day they arrive.

You stay in charge.

Only what you needForms ask for what the job needs and nothing more. Answers stay in your records, not in someone's inbox.
Concerns reach you firstAn unhappy answer is sent straight to you, so you can call before it turns into a public review.
Your questions, your wordsYou approve every form before it goes live.
Confirmations only confirmThe only message sent on its own is a note that the form was received.

Questions owners ask.

How many forms can I have?

As many as the business needs. Most start with two or three: intake, an order or request form, and a feedback survey.

Can people upload photos or files?

Yes. Photos of a job, an insurance card or a document attach to their record.

Do customers need to download anything?

No. Forms open in any phone or computer browser.

Can I send a survey after every visit?

Yes. It's one of the most useful forms there is, and it pairs well with Get more 5-star reviews.

Can I cancel?

Yes, any month. Setup is a one-time fee.

Stop retyping what customers already told you.
